I have gestational diabetes and a 36-week pregnancy, I am on insulin therapy, and fasting sugar is still high. What to do?

Hello! I have a pregnancy of 35-36 weeks, the diagnosis of HSD, from 15 weeks on insulin therapy - Levemir for the night, now a dose of 22 for the night, but the glucose results on a fasting blood glucose meter are 5.5 and 5.8 for the 5th day already. Is it possible to increase the insulin dose overnight or is it better to increase and divide it in half in the morning and evening? After a meal during the day, glucose is always within the normal range and was. Thank!
Natalia, 38 years old

Hello, Natalia!

Dose 22 is medium, and insulin Levemir is active for 16-17 hours, with high doses, up to 24 hours.

That is why most often we prescribe Levemir 2 times a day - in the morning and in the evening. In pregnant women, fasting sugar, to which we aspire, is up to 5.1 mmol / l, 2 hours after eating, up to 7.1 mmol / l. Accordingly, it is better to change insulin therapy, that is, it is better to break the introduction of Levemir into 2 injections, but this should be done after the doctor sees your sugar diaries (all sugars during the day) and the nutrition diary - XE distribution during the day, as in pregnant women women, we always change therapy with caution so as not to harm the child.

Patients with GSD (that is, pregnant women) in polyclinics have the right to go to the endocrinologist for an appointment without a turn. If you have any questions, it is better to go to the clinic immediately, so as not to wait for answers.
